Sign your repair authorization

I authorize my insurance company to pay All Colors LLC, 2153 Sutherland Ave, Knoxville, TN 37919, directly for all repairs performed on the vehicle described above. I appoint All Colors LLC and its authorized representatives as my attorney-in-fact for the limited purpose of receiving and endorsing on my behalf any check, draft, or insurance payment issued for these repairs, and depositing it to the account of All Colors LLC to be credited against my repair invoice. I understand this direction to pay does not relieve me of responsibility for my deductible or for any amount my insurance company does not cover.

I authorize All Colors LLC to perform the repair work described in the estimate for this vehicle, along with all necessary parts, materials, and sublet services. I grant permission for employees of All Colors LLC to operate the vehicle on streets and highways for the purpose of inspection, testing, and delivery. I understand that additional damage is frequently discovered after disassembly, and that supplements to the estimate may be submitted to my insurance company. I understand the shop is not responsible for loss or damage to the vehicle or to articles left in it caused by fire, theft, weather, vandalism, or any other cause outside the shop's direct control, or for delays caused by parts availability, shipping, or insurer approval. An express mechanic's lien is acknowledged on the vehicle described above to secure the amount of the repairs performed.

I understand that All Colors LLC is not responsible for personal property left in the vehicle, including but not limited to electronics, tools, unmounted stereo equipment, firearms, cash, medication, and documents. I have removed, or accept full responsibility for, all personal items in the vehicle.

I agree that payment in full — including my insurance deductible and any amount not covered by my insurance company — is due before the vehicle is released. I understand that All Colors LLC may charge customary daily storage fees for vehicles not picked up within ten (10) days after I am notified that repairs are complete. I acknowledge that the estimate may include aftermarket, reconditioned, or recycled parts when specified by my insurance company, and that I may request original equipment parts at additional cost to me.
